To get a comprehensive list of what you can enjoy as a member, go to the homepage and click on the Providers and Fee Schedule. This link will give you a good idea of what are services that you can enjoy and how discounts you can get from these services. Who can qualify for the family plan? People who belong to the same household can qualify for the family plan. There is no age limit required not is it necessary that these people are related to each other. As long as these people belong to the same household, the can qualify for the family plan. Can I use the services of other healthcare service providers in another state? Yes you can. As long as this healthcare service provider is a member of the AmeriPlan USA® network, you can use its services. How discounts can I get for dental services? You can get as much as 80% on preventive work (teeth cleaning, x-rays, etc.) performed by a general dentist. For all restorative and cosmetic work (fillings, crowns, braces, etc.), you can get around 25% - 65% discount. You can also get 25% discount on specialist fees. Is there a long term contract lock agreement involved in the membership? No. There is no such condition involved. How to I pay my membership? You can opt to pay your AmeriPlan® membership monthly, quarterly or annually. If you choose to pay annually, you will get a 20% discount on the membership fee. How do I cancel my membership? If you no longer wished to be an AmeriPlan®, just notify the company of your intentions 30 days in advance. How does AmeriPlan® select their healthcare services providers? The company actively recruits new members to its network. It also acts on the referrals from its members. All service providers that are considered as potential partners of the company are subjected to evaluation. To become a member of the AmeriPlan® network, one should have an active license to practice in the State where he or she is located. Furthermore, the healthcare must have professional liability insurance. AmeriPlan® conducts onsite inspection and extensive license checking to ensure that all its healthcare service providers can give the best services to its members. How Do I know that I am really getting discounts from dental services? All AmeriPlan® are given a list of dental procedures with it corresponding Usual and Customary Fee (UCF). The list will also include the discounts given for the different types of procedures. However, for some dental services such as the Invisalign Braces and orthodontics in process, AmeriPlan® may not be able to provide the members with the exact amount of fees. When and how often can I go to see the dentist or chiropractor? You can go to the dentist or the chiropractor anytime. There are no limitations on number of visits. If I have concerns over the fees and the quality of service of the healthcare provider, can I file a complaint? If you have concerns on the fees and the quality of services given by the healthcare provider, you may call the AmeriPlan® Customer Service and file a complaint. The Patient/Provider Liaison shall take care of disputes on fees, quality of service and others. If the matter is not resolved after the intervention of the Patient/Provider Liaison, the complaint will be forwarded to State Professional Board for investigations and further actions. Will I need to pay additional amount of money for membership after becoming a member? No. You memship payments will be fixed for the next two years after you sign or renew your membership. Is Lasik Eye Surgery covered under the Ameriplan®? Yes. Ameriplan® offers up to 55% discount on this procedure. Please Contact Us for details.
